When Baby’s giggles are not funny: seven years overlooked diagnosis in a case of gelastic seizures

چکیده
L aughter actually has positive emotional effect. However, it could be pathologic when laughter is not related to emotion and occurs independent of a stimulus in the environment. We reported a 7-year-6-month old Thai boy who presented with ‘early giggles’ since the age of 2 months. Despite parental concerns, the symptoms were overlooked until seven years of age, when he developed complex partial seizures. An MRI study of the brain revealed a small ill defined lesion within the tuber cinereum that was compatible with hypothalamic hamartoma. We also reviewed and emphasized the characteristics of the pathologic laughing in infancy that should not have been ignored. Early diagnosis leads to appropriate counseling and proper seizure management. Common comorbidity such as precocious puberty and cognitive impairment or behavioral impediment should be followed up closely in long-term care.

Gelastic Seizures in a Patient with Right Gyrus Cinguli Astrocytoma
OBJECTIVE AND IMPORTANCE Gelastic seizure (GS) also known as "gelastic epilepsy" is a rare type of seizure associated with several different conditions such as tumors--hypothalamic hamartromas, tuberous sclerosis, hemangiomas, post infectious foci, cortical temporal dysplasia. We report one case of this rare condition generated by a right gyrus cinguli gr. II astrocytoma. CLINICAL PRESENTATIO...

We report a case of hypothalamic hamartoma in an adult female who presented with gelastic seizures, generalized convulsions, and ictal aggressive psychotic behavior. Anticonvulsant treatment was ineffective in controlling the epileptic seizures. Surgical excision after accurate imaging diagnosis 3 decades after the onset of symptoms markedly ameliorated her condition. Delayed and erroneous diag...

Gelastic seizures are an uncommon seizure type. They are most frequently observed in patients with hypothalamic hamartoma. Their association with other types of cerebral lesions is rare. Depending on the location of the lesion, gelastic seizures may or may not be accompanied by a subjective feeling of mirth. The pathophysiological mechanisms of this type of seizure are still undefined, and litt...

PURPOSE To delineate the clinical spectrum and patterns of evolution of epilepsy with gelastic seizures related to hypothalamic hamartoma (HH). PATIENTS AND METHODS We evaluated patients with HH, observed between 1986 and 2002 for whom at least one ictal video-EEG or EEG recording of gelastic seizures was available. RESULTS Six subjects (four male, two female) with sessile HH between 0.8 an...

Gelastic seizures are often associated with hypothalamic hamartomas. However, focal cortical dysplasias can also cause “laughing seizures”, and such cases can be difficult to localize with EEG. This case report presents a 29-year-old woman who was successfully rendered free of gelastic seizures after resection of a frontal cortical dysplasia, localized through MRI and SPECT imaging.
